than asthma or allergy<strong> (i.e. Motrin, Tylenol) <\/strong>you will need to have your physician fill out a<strong> Medication Administration Form.\u00a0*** A parent or guardian will need to bring the form along with the medication to the clinic.<\/strong><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t<div class=\"elementor-column elementor-col-33
